Does anyone know how to order custom clothing?

I wanna know if there are any companies (online or walk-in) that make entirely custom clothing. I'm not just talking custom embroidery or sizing but complete customization, including stuff like purposeful tears and stuff you wouldn't consider "ordinary" like spikes and tall necks (think a vampire cloak) as well as special colors, etc... It'd be nice to know since this kinda stuff interests me. I'd appreciate the help, thanks.
