Where can I find replacement crochet afghan pattern Kaleidoscope, designed by Mary B Caldwell. Help?

I don't remember which company put the pattern out.. I lost the pattern( folder) it was not in a book. I can supply pic of work completed to this point, for anyone who might be able to help. How do I contact a person to send them a picture of the pattern?
